What the Program Does
The Community Paramedic Program expands the role of paramedics beyond emergency response. This program focuses on residents who may need extra support managing medical conditions, navigating the healthcare system, or addressing recurring non-emergency issues.
Through scheduled visits, education, referrals, and follow-up, Community Paramedics work to prevent crises before they occur and improve overall quality of life within the community.
Who We Serve
- Residents who frequently call 911 for non-emergent needs
- Seniors at increased risk for falls or home safety issues
- Individuals managing chronic medical conditions
- Residents needing assistance accessing healthcare or community resources
Services We Provide
Scheduled check-ins to identify concerns early and support overall health goals.
Home safety evaluations and fall risk reduction strategies.
Support for conditions such as diabetes, hypertension, COPD, CHF, and asthma.
Education and support to improve medication understanding and adherence.
Assistance connecting with primary care, specialty care, and social services.
Narcan education, overdose prevention, and drowning prevention outreach.
